Clinical impact of a pharmacist + health coach chronic disease management program in a rural free clinic

ObjectivesRecent data have demonstrated benefits of pharmacist-led protocols for chronic disease state management in the primary care setting. Health coaching has also been shown to improve patient outcomes and reduce health costs. A program was initiated August 2017 at a rural, free clinic provide team-based, patient-centered through use pharmacist-provider collaborative practice patients with diseases such as diabetes, hypertension, hyperlipidemia.MethodsAfter an initial examination, physicians could refer by pharmacist + coach team. Patients continued see their provider least yearly needed. The team provided protocol-based approach management, well education pertaining diet lifestyle recommendations. In-depth medication were each visit. Motivational interviewing conducted Clinical metrics collected baseline analyzed routinely after initiation, including glycosylated hemoglobin (A1c), blood pressure, lipids. Primary objectives evaluate program’s impact on A1c, cholesterol outcomes.ResultsA total 95 included analysis (A1c n = 37; systolic diastolic pressure 47; 40; low-density lipoprotein [LDL] 38; high-density triglycerides 40). From 1 year, statistically significant improvements observed A1c (mean ± standard deviation, 8.55 2.58 7.04 1.12, P < 0.001), (136.79 20.04 123.15 16.81, (87.94 12.28 78.64 10.98, (198.25 52.47 183.55 47.22, 0.014), LDL (115.74 43.56 105.92 39.27, 0.040).ConclusionA protocol-driven clinical conjunction registered nurse low-income, setting improved cholesterol, cholesterol.
